Trying to help a kid diagnose this over the phone, lol.

Had below freezing temps last night and when he fired it up it sounds like he has a radical camshaft with lots of lope.
Once it warms up it goes away.

Could it be simple condensation?
Slight intake leak that seals itself once things warm up and expand?

I’m not familiar with known issues of these engines other than they like to spit spark plugs.

Any other ideas here?

Is the check engine light on? Could be a bad IAT or ECT sensor that hasn’t been bad long enough to trigger an alert.
Does he have a code reader?

IAC Maybe.
Idle air control valve sticking. Back of the throttle body near the firewall. He needs to give it a tap next time before he starts it.

Had it been sitting all weekend, and what oil does he have in it? I have a 5.4 at the the river that does this, best I can figure is bleed down. Will be fine if I start it up every day, let it sit three and it's like a massive vacuum leak.

Idle air control is bad. When starting hold the throttle slightly open if it starts but does not idle the IAC is bad. The Pintile valve gets carboned up and won’t idle. Sometimes you can start and rev the motor several times and it will start to idle again, but will carbon up again. Know problem with the module motor platform. I changed many IAC when I worked for the PD, cops didn’t like their Cars not idling!

I have had 2 V10's

Had a similar noise but not sure it's the same as yours. But, a mechanic buddy said it could be the lifters making noose until it gets warm. I am in HB so the cold isn't even close to freezing.

My 2018 F150 has and had a cam phaser problem that causes a rattle when starting if its cold outside.

Could it be the power steering pump rattling at startup?
Next time they start the truck, move the steering wheel and see if the sound goes away or changes.

Mine is going in for the second time. Glad I have the extended warranty.

What did they do the first time?

This is my 2nd time. First time was just a reflash, that I figured would do nothing, and did nothing. Round 2 is replacing the phasers under warranty.

Some of them have a big rubber hose (maybe for the IAC bypass?...dunno, been a while) right under the throttle body that rots and creates a vacuum leak. Obviously vacuum leaks will have more pronounced symptoms when it's cold. I'd check there.

Does he smoke? If so, tell him next cold morning before he starts it, find a vacuum line and pull if off of the non-manifold side...then take a nice big drag and blow all the smoke into the hose...keep an eye open while doing that and see if smoke leaks out anywhere.

Surging is usually a sticking/stuck/failing IAC (idle air control valve). Can also be a TPS (throttle position sensor). Possibly a NTC2 (coolant temp sensor for ECU) Only in warm up phase would make me look there first.

Strangely enough, if it's really low on coolant, it can do the same. Since you are talking about low temps, I would ask if it has the proper 50/50 ratio of coolant? If it's water and freezing can cause weird issues as well.

If you had a scan tool that could stream data, and someone capable of analyzing it, you can find it quickly.

Have a 3,5 honda in my buggy last year after putting trans back in i fired it up it cranked a litttle long but damn think sounded like a top fuel engine then rveed and came down to ide rev idle etc.Shut off wouldnt start plugs wet.Luckily set a code and showed ect sensor i disconnected pulled heated it checked ohms it seemed fine re inistalled everything was perfect again.

Guess somehow it didnt read at first and acted like it was real cold and threw in extra fuel.

The Excursion section on Ford forums coughed up multiple threads with the same issue…IAC.
I’m not sure?
The next morning it started just fine. 🤷‍♂️

He said he was on some Excursion forum and several people told him it’s “normal for that to happen from time to time” lol.

Must be a Ford thing😎
I post over there and saw numerous threads about the same issue...IAC.

I changed mine out a few years back trying to chase down a random stall issue....but even so, mine also runs a little rough at startup, and then idles smooth after putting it in gear.
