pt cables could have slipped have they checked any in or effected area ?
They have very aggressive "dogs" / wedges with serrated teeth that bite in to the tendons when the hydraulic rams are backed off. If the wedges didn't take during stressing there is no chance they failed unless the tendon was drilled through or extremely over stretched with settlement or a major miss calculation in structural engineering.

Almost 10 times out of 10 it isn't the builders issue FYI. Builders build to what the developer / owners design / engineering team (owner contracted and paid for) draw up and GC's / subcontractors price. Commercial buildings are rarely design build on the structural part, developers / owners are to cheap to put open ended contracts out like that unless the site is a disaster waiting to happen and they are trying to limit their liability.
