It should be by choice. Municipal water systems in most states are required to fluoridate the water for the public. One of my friends/customers maintains and also owns some water companies here in AZ. They are not required to put fluoride in the water. He said the ppm required is pretty precise. If you over shoot, you can do serious harm. As a provider, he doesn't like the idea. As a father, he didn't seem to mind either.

My kids have basically spent majority of their lives on well water. One of the three has had one cavity. We've gone back and forth with fluoride type toothpaste over the years though, so can't say if that made a difference.
